Fort Wayne Medical Education Program has agreed to settle a class action lawsuit concerning a cyberattack in December 2024 that allegedly exposed personally identifiable information and protected health information of 29,485 individuals. Class members can claim reimbursement for out-of-pocket losses up to $500, lost time up to $80, or extraordinary losses up to $5,000. Alternative cash payments of $40 are available. Group 1 members can receive credit monitoring, and Group 2 members can receive medical monitoring services. The claim deadline is October 27, 2026.

You may be eligible if you:

• Individuals whose private information was compromised in the December 2024 FWMEP data breach and are included in the settlement class list.
• Group 1: Individuals whose Social Security numbers were potentially compromised.
• Group 2: Individuals whose Social Security numbers were not compromised but other private information was affected.

If you do not have every document, you may still be able to file. Many settlements accept claims without receipts.
