This settlement addresses allegations that DaVita Inc. failed to adequately protect patient and customer data from a ransomware attack in April 2025.

DaVita Inc. has agreed to a $15 million class action settlement to resolve claims related to a criminal ransomware attack in April 2025 that potentially exposed the personal and clinical information of patients and customers. The lawsuit alleged DaVita failed to adequately protect this data, which may have included names, addresses, Social Security numbers, health insurance details, health conditions, treatment information, dialysis lab test results, tax identification numbers, and in limited cases, images of checks. Class members can claim up to $2,500 for documented out-of-pocket losses resulting from identity theft or fraud, or elect to receive a pro rata cash payment (estimated at $50) and three years of credit monitoring services.

You may be eligible if you:

• Current and former patients and customers of DaVita Inc. in the United States whose personal and clinical information was potentially accessed in the April 2025 ransomware attack.

• Basic contact information
• Dates tied to your purchase or account activity
• Proof of purchase or supporting documents

If you do not have every document, you may still be able to file. Many settlements accept claims without receipts.

• Complete and submit the online claim form or the PDF claim form by mail.
• Provide your unique ID and PIN from the settlement notice.
• For out-of-pocket losses, include supporting documentation.
• The claim deadline is January 19, 2027.
