The breach allegedly exposed sensitive personal and medical information, including names, dates of birth, addresses, Social Security numbers, billing and claims details, diagnosis and clinical information, phone numbers, email addresses, and health conditions on FMLA forms. Individuals who received a notice of the breach and whose information may have been compromised are eligible to file a claim.
