Alleged transmission of patient information to third parties via website technologies on the Boone Health public website.

Boone Health Inc. and CH Allied Services have agreed to settle a class action lawsuit alleging they transmitted patient information to third parties via website technologies on their public website. Current and former patients who logged into the Boone Health patient portal between April 1, 2021, and Dec. 5, 2022, and resided in Missouri during that period may qualify for a $20 cash payment and one year of Privacy Shield Pro digital privacy monitoring and identity protection. A claim must be submitted by November 9, 2026. To file a claim, provide the login ID and password from your settlement notice.

You may be eligible if you:

• Current or former patients of Boone Health who logged into the patient portal at least once between April 1, 2021, and Dec. 5, 2022.
• Resided in Missouri during the qualifying period.

• Basic contact information
• Dates tied to your purchase or account activity
• Proof of purchase or supporting documents

If you do not have every document, you may still be able to file. Many settlements accept claims without receipts.
